If you intend to purchase only one (1) dog tag for the exhibit (which includes the Service Member’s/Veteran’s dog tag, 6” dog tag chain, and laminated photo, as well as a memento dog tag with optional chain), please click on the Express Online Payment box below.

Option B: Fillable Form & Online Payment

Option B allows you to purchase multiple dog tag sets for the exhibit (which includes the Service Member’s/Veteran’s dog tag, 6” dog tag chain, and laminated photo, as well as a memento dog tag with optional chain).

WIth Option B, you will be able to make payment now and email your order forms and photos by following the steps below.

Step 3: Email the optional photo of the veteran/service member to: alwaysfaithful@mclwilco1022.org.

Note: Be sure to include both your name and the name of the veteran/service member in the email so the photo can be matched with the order form and donation, once received.

Step 4: Mail the downloadable order form, with check or money order, to the address noted in the lower-right hand corner of the downloadable order form.
